Nomination Guidelines

Criteria & Award Selection

The C. Edward Pleasants, Jr. Lifetime Achievement Award seeks to recognize individuals who have had a profound impact on Forsyth County and its people. Individuals chosen for the C. Edward Pleasants Lifetime Achievement Award will:

  • Possess a passion for Winston-Salem/ Forsyth County;
  • Have made a profound and lasting impact on Forsyth County and its people;
  • Demonstrate leadership in civic organizations and community initiatives;
  • Exemplify visionary community leadership that has inspired others;

One honoree will be chosen by a committee comprised of board members of Leadership Winston-Salem. The potential award recipient will be contacted by phone and must agree to accept the award on June 6 at Leadership Winston-Salem’s Celebration event at The Millennium Center.

The C. Edward Pleasants, Jr. Lifetime Achievement Award will support a grant of $25,000 to a Forsyth County charity (or up to four charities) chosen by the awardee, in recognition of this special honor. The award is made from the C. Edward Pleasants Lifetime Achievement Award Fund which is held at The Winston-Salem Foundation.

Eligibility

  • Nominee must live in Forsyth County, North Carolina.
  • Self-nominations are accepted.
  • Family members may nominate other family members.
  • Awards will not be given posthumously.
  • Nominations must include the nominees’ name, address, telephone number, and email address.
  • The person submitting a nomination must include their name, address, telephone number, and email address.

Entries

  • Entries are accepted starting 12:01 AM Eastern Standard Time (EST) on April 1, 2024 and ending on
    11:59 PM (EST) on May 1, 2024.
  • Submitting entries:
    • All sections must be completed.
    • All entries must be submitted online.
  • All applications must be submitted by May 1, 2024 at 11:59 p.m.
  • Supporting documentation such as photographs, news articles, etc. will not be accepted; award selection will be based solely on information provided on nomination forms.
  • Limit three entries per person. All entries become the property of Leadership Winston-Salem and will not be returned.
  • Leadership Winston-Salem is not responsible for any lost, late, delayed, illegible, incomplete, mutilated, postage due or misdirected entries or mail. Leadership Winston-Salem assumes no responsibility for any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft or destruction, unauthorized access to, or alteration of entries. Leadership Winston-Salem is not responsible for any problem or technical malfunctions of any telephone network or lines, computer online systems, servers or provider, computer equipment, software, failure of any entry to be received by Leadership Winston-Salem on account of technical problems or delays, including any injury or damage to participant’s or any other person’s computer related to or resulting from participation or downloading any materials for this award.
  • Your entry will be disqualified if you (i) provide information that is incomplete, inaccurate or fraudulent; or (ii) fail to comply with or circumvent the terms and conditions of these Official Nomination Guidelines.
  • Leadership Winston-Salem reserves the right, in its sole discretion; to cancel, modify, or suspend any portion of these awards should any causes beyond the control of Leadership Winston-Salem corrupt the administration, security or proper participation or entry submission.
